p27和ILK在胃癌及癌前病变中的表达及意义 被引量:16

摘要 目的:探讨p27和ILK在胃癌及癌前病变中的表达及其与胃癌病理参数之间的关系。方法:用免疫组化技术(SP法)对正常胃黏膜(normal gastric mucosa,NGM)、慢性浅表性胃炎(chronic superficial gastritis,CSG)、慢性萎缩性胃炎(chronic atrophia gastritis,CAG)伴肠上皮化生、慢性萎缩性胃炎伴非典型增生各20例和胃癌(gastric carcinoma,GC)60例标本进行免疫组织化学染色,分析p27和ILK的表达与胃癌临床和病理的关系。结果:p27和ILK表达阳性率分别为:NGM组100%和5%,CSG组85%和10%,CAG伴肠化组70%和20%,CAG伴非典型增生组45%和30%,胃癌组38.3%和40%。胃癌组和CAG伴非典型增生组p27阳性率显著低于其他组(P<0.05),ILK阳性率则显著高于其他组(P<0.05)。p27和ILK在胃癌中的表达与肿瘤分化程度、浸润深度及肿瘤临床分期相关,p27蛋白的表达还与有无淋巴结转移相关。p27和ILK在胃癌中的表达呈显著负相关(r=-0.768,P<0.05)。结论:检测胃癌组织中p27和ILK的表达有助于判断肿瘤的进展程度,两者联合检测有助于判断肿瘤预后。 Objective:To investigate the expression of p27 and ILK in gastric carcinoma (GC) and precancerous lesion and their correlation with the clinicopathological parameters. Methods: Immunohistochemical staining was used to evaluate the expression of p27 and ILK in the samples of normal gastric mucosa ( NGM, n = 20), chronic superficial gastritis ( CSG, n = 20 ), chronic atrophic gastritis (CAG) with metaplasia ( n = 20), CAG with dysplasia ( n = 20) and GC (n = 60), and their correlations with the clinicopathological parameters were analyzed. Results: The positive rates of p27 and ILK expression were 100% and 5% in NGM,85% and 10% in CSG,70% and 20% in CAG withmetaplasia,45% and 30% in CAG with dysplasia, and 38.3% (23/60) and 40% (24/60) in GC, respectively. The positive rates of p27 expression in GC and CAG with dysplasia were lower than that in NGM, CSG, and CAG with metaplasia ( P 〈 0.05 ), and the positive rates of ILK expression in GC and CAG with dysplasia were higher than that in NGM, CSG, and CAG with metaplasia( P 〈 0.05 ). The expression of p27 and ILK were significantly correlated with tumor differentiation, infiltration and clinical stages, and p27 expression was also correlated with lymph nodes metastasis. Furthermore, the expression of p27 was inversely correlated with ILK expression in GC (r= -0. 768, P 〈 0.05 ). Conclusion: It is helpful to detect p27 and ILK protein expression in the judgment of GC progression, and combined analysis of them is of added prognostic value.
