“医院-社区-家庭”护理干预模式在老年糖尿病患者中的应用体会

Application of the "Hospital-Community-Family" nursing mode for elderly patients with diabetes

目的探讨提高老年糖尿病患者自我管理能力的有效途径。方法 2008年1月1日开始我科实施"医院-社区-家庭"护理干预模式,采用自制问卷进行调查,评价患者1年后的糖尿病自我管理能力。结果干预组患者的糖尿病自我管理能力及糖化血红蛋白达标高于对照组患者,差异均具有统计学意义(P<0.01)。结论 "医院-社区-家庭"护理干预模式有助于提高老年糖尿病患者的自我管理能力。

Objective To explore the effeetive method to improve the self - management ability of elderly patients with diabetes. Methods The ＂ Hospital - Community - Family＂ uursing mode was implemented in our department from Jan 1,2008. The ability of diabetes self - management was assessed with a self - designed questionnaire. Results After the intervention,the ability of diabetes self- management and HbAlc compliance in the intervention group were higher than those of the control group （P 〈0.01 ）. Conclusion The ＂ Hospital - Community - Family＂ nursing intervention mode can improve the self- management ability of elderly patients with diabetes.
