子宫肌瘤介入治疗的临床分析

The clinical analysis of uterine artery embolization of hysteromyoma

目的探讨子宫肌瘤动脉栓塞术前、后性激素变化及闭经原因。方法对183例子宫肌瘤进行子宫动脉栓塞治疗,测定栓塞前后的促卵泡素(FSH)、黄体生成素(LH)及雌二醇(E2);观察术后闭经情况。结果177例未闭经患者,促卵泡素术前8.72±3.23IU/L、术后6月9.81±1.92IU/L,促黄体生成素术前11.72±6.21IU/L、术后6月13.58±2.31IU/L,雌二醇术前162±56pg/ml、术后6月185±76pg/ml。栓塞前后促卵泡素、黄体生成素及雌二醇没有统计学上的差异(p>0.05,t检验);6例术后出现闭经患者性激素变化较明显。结论在双侧卵巢支未被完全异位栓塞时,子宫肌瘤动脉栓塞术对性激素没有明显影响,但对双侧卵巢供血动脉的栓塞可导致闭经。

Objective To investigate the changing of serum hormone before and after uterine artery embolization（UAE） for uterine fibroids and reasons of amenorrhea after UAE.Methods 183 cases of uterine fibroids were performed uterine artery embolization.In all cases,follicle stimulating hormone（FSH）,luteinizing hormone（LH） and estriol（E2） were measured before and after the UAE.The symptoms of amenorrhea were observed after UAE.Results In 177 cases of patients without amenorrhea,the serum level of FSH before embolization was 8.72±3.23 IU/L,9.81±1.92 IU/ L 6 months after embolization.The serum level of LH before embolization was 11.72±6.21 IU/L,13.58±2.31 IU/L 6 months after embolization.The serum level of E2 before embolization was 162±56pg/ml,185±76pg/ml 6 months after embolization.There was no statistical difference between the measurement of FSH,LH,E2 before and after UAE（p〉0.05,t-test）.But FSH,LH and E2 appear the change of menopause in 6 cases.Conclusion If the bilateral ovarian branches of uterine artery have not been embolized,UAE would not affect the serum level of FSH,LH and E2 6 months after embolization.The embolization of the bilateral ovarian blood supply artery can lead to amenorrhea.
