淋巴结原发性上皮样血管内皮瘤临床病理观察

Primary epithelioid hemangioendothelioma of lymph node:a clinicopathologic observation

目的探讨淋巴结原发性上皮样血管内皮瘤(EH)的临床病理学特点及鉴别诊断要点。方法分析1例淋巴结原发性上皮样血管内皮瘤的临床、组织病理学资料,进行常规HE染色及免疫组化检测,并结合文献进行讨论。结果组织学上,肿瘤细胞呈圆形或类圆形上皮样,瘤细胞增生呈乳头状凸向腔内,胞质嗜伊红染,细胞边界不清。免疫组化示肿瘤细胞CD31(+),CD34和FⅧRAg(-)。结论淋巴结原发性上皮样血管内皮瘤是罕见的低度恶性血管性肿瘤,可出现多种组织结构,有一定的特征性,应与淋巴结转移性恶性肿瘤及原发于淋巴结的其他血管源性肿瘤鉴别。

Objective To study the clinicopathological features and differential diagnosis of pritnary epithelioid hemangioendothelioma of lymph node. Methods A case of primary epithelioid hemangioendothelioma of lymph node was investigated by hematoxylin-eosin, immunohistochemical staining with analyses of clinic and histopathology data, and review of the related literature. Results Microscopically, the papillary hyperplasia of tumor cells were outstretched into cavities, which appeared round and oval epithelioid, and the cytoplasm of the tumor cells was eosinophilic, with unclear rim; histochemieally, the tumor cells was positive for CD31, while negative for CD34 and F Ⅷ RAg. Conclusion Primary epithelioid hemangioendothelioma of lymph node is a rare low-grade angiosareoma, and it could appear various morphological characteristics, and should be differemtiated from metastatic carcinomas and primary hemangioma in lymph node.
