糖尿病肾病患者血浆黏附分子水平的探讨

Variations and significance of hematoplasma adhesion molecule in diabetic nephropathy

摘要 目的探讨糖尿病肾病(DN)患者血浆黏附分子P-选择素的水平变化及其临床意义。方法采用酶联免疫吸附试验(ELISA)法检测了30例糖尿病并发肾脏微血管病变患者血浆P-选择素水平,同时对24h尿蛋白排泄率(UAER)进行了测定,20例无肾病并发症糖尿病患者和17例正常体检者做为对照。结果①糖尿病并发肾脏微血管病变患者组及无肾病并发症糖尿病患者组血浆黏附分子P-选择素水平均显著高于正常对照组(P<0.01,P<0.05),糖尿病肾病患者组黏附分子P-选择素水平显著高于无肾病并发糖尿病对照组(P<0.01);②黏附分子P-选择素水平与UAER呈正相关(r=0.81;P<0.001)。结论血浆黏附分子P-选择素与糖尿病肾病的发生、发展密切相关,为糖尿病肾病的诊疗及病情监测提供了新思路。 Objective To investigate the changes of hematoplasma adhesion molecule in diabetic nephmpathy and its clinical significance. Methods Enzyme linked immunosorbent assay was used to detect the level of P-selectin in 30 diabetic patients comphcating with kidney microangiopathy. Twenty simple diabetics and 17 cases of health volunteers were taken as control. Urine protein excretory rates were also detected as reference. Results The levels of blood' plasma adhesion molecule P-selectin in all diabetics were significantly higher than health controls (P〈0.01 ,P〈0.05);Levels in diabetics complicated with kidney microangiopathy were significantly higher than that in simple diabetics (P〈0.01);The levels of hematoplasma adhesion molecule was positively correlated with urine protein excretory rate (r=0.81;P〈0.001). Conclusion Levels of blood plasma adhesion molecule was closely related to the nosogenesis and developmen of diabetic nephropathy.
