鼻内窥镜下经鼻泪囊鼻腔造孔术的应用分析

Application of Dacryocystorhinostomy by Nasal Endoscope

目的探讨在基层医院应用简易的鼻内窥镜下的泪囊鼻腔造孔术治疗慢性泪囊炎的疗效及安全性。方法对宁化县医院自1999年5月以来开展鼻内窥镜下鼻内泪囊鼻腔造孔术治疗慢性泪囊炎48例(55眼)患者的临床资料进行回顾性分析,观察其临床疗效及并发症发生情况。结果治愈49眼(89.1%),好转5侧(9.1%),无效1侧(1.8%),总有效率98.2%。1例无效者经扩大骨窗清除肉芽后症状消失,泪道通畅。所有病例均经6个月以上随访,无并发症发生,效果满意。结论本术式方法简单,疗效显著,安全性好,在基层医院容易开展。

Objective To analyze the clinical feasibility and efficacy of dacryocystorhinostomy in treating chronic dacryocystitis by the simplex and easy nasal endoscope at the primary hospital. Methods Dacryocystorhinostomy by nasal endoscope has been carried out in 48 cases（55 eyes） patients with chronic dacryocystitis since May 1999 in our hospital. The patient＇s clinical data were retrospectively analyzed to observe the clinical efficacy and complications occurrence. Results 49 eyes （89.1% ） were cured,5 sides（9.1% ） were improved,one side was ineffective（ 1.8% ） ,the total effective rate was 98.2%. After the bone win- dow of ineffectivel case was expanded and granulation was removed, clinical symptoms disappeared, lacrimal duct became unobstructed. The follow up more than six months showed that no complications took place, and the clinical effect was satisfactory. Conclusion The surgical method is simple,effective and safe. The clinical effect is notable and easy to be carried out in to the primary hospitals.
