摘要 目的:采用^(18)F-脱氧葡萄糖(FDG)PET/CT脑显像技术探讨无脑部疾患的原发性胰腺癌患者的大脑静息状态下的葡萄糖代谢规律及其临床应用价值。方法:19例无脑部疾患的原发性胰腺癌患者进行了常规全身^(18)F-FDG PET检查,采用统计参数地形图分析软件(SPM)回顾性分析了其脑部^(18)F-FDG PET影像资料,并与年龄、性别相匹配的60名健康体检者进行了比较。结果:SPM分析显示无脑部疾患的原发性胰腺癌患者和对照组相比,其大脑在静息状态下出现的葡萄糖代谢减低区主要分布在左侧额中下回,两侧颞叶也有代谢减低区;静息状态下葡萄糖代谢升高的区域主要见于左侧杏仁核和右侧海马周围。结论:胰腺癌患者静息状态下的脑代谢非常符合重症抑郁发作或双极抑郁的代谢特点,这些代谢的改变有望应用于胰腺癌患者精神情感障碍的客观评价指标。 Purpose:To explore the change of regional brain metabolism in pancreatic carcinoma patient by ^18F - fluoro - deoxyglucose positron emission tomography(^18F - FDG - PET) during resting state. Methods: Nineteen primary pancreatic carcinoma patients without brain metastasis underwent whole body PET during resting state for the purpose of diagnosis or staging. The acquired brain data were retrospectively compared to the data of 60 age and gender - matched controls, which had the whole body PET/CT for health checkup. Data were analyzed using statistical parametric mapping (SPM2) software. The hypo - metabolic and hyper - metabolic areas were identified by MNI Space Utility (MSU) software. Results: Compared with the control group, hypo - metabolic brain areas in the pancreatic carcinoma patients were observed mainly in the left superior and middle frontal gyrus, bilateral inferior and middle temporal gyrus, and the left precuneus. Hyper - metabolic areas were also observed in the left amygdale and the right peri - hippocampus areas. Conclusion: Persistently over activity in the left amygdale and lower frontal cortex metabolism, especially during resting state, implied intensive major depressive episodes in pancreatic carcinoma patients.
