经阴道三维超声在官腔粘连诊断中的价值

Three-dimensional transvaginal ultrasonography in diagnosis of intrauterine adhesion

对78例患者分别进行经阴道三维、二维超声检查，与宫腔镜检查结果比较，对经阴道三维超声诊断宫腔粘连（IUA）的价值、IUA超声分级进行分析。经阴道三维超声诊断IUA的敏感性97％（58／60），特异性18／18，准确性97％（76／78），阳性预测值100％（58／58），阴性预测值90％（18／20）。经阴道三维超声较二维超声诊断的准确性明显提高（P〈0．01）。IUA的超声分级和官腔镜分级有较好的相关性。经阴道三维超声检查对IUA临床治疗选择有指导意义。

Seventy eight patients suspected of intrauterine adhesion （IUA） were scanned with twodimensional （2D） and three-dimensional （3D） transvaginal ultrasonography, the results were analyzed with hysteroscopic findings as the gold standard. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of three-dimensional transvaginal ultrasonography in diagnosis of IUA were 97% （58/60）, 18/18, 97% （76/78）, 100% （58/58） and 90% （18/20）. Three-dimensional transvaginal ultrasonography was more accurate in diagnosis of IUA than two-dimensional uhrasonography （ P 〈 0. 01 ） , and the IUA classification and grading with three-dimensional transvaginal uhrasonography was better correlated with hysteroscopic findings.
