耐碳青酶烯类鲍氏不动杆菌医院获得性肺炎临床分析

Hospital-acquired Pneumonia Caused by Carbapenem-resistant Acinetobacter baumannii:A Clinical Analysis

目的了解耐碳青酶烯类鲍氏不动杆菌医院获得性肺炎的特点,总结治疗经验。方法回顾性地分析12例鲍氏不动杆菌肺炎患者的临床表现、药敏及治疗方法。结果耐碳青酶烯类鲍氏不动杆菌肺炎均发生于有基础疾病的患者,胸片多表现为双侧斑片状渗出影;分离菌对亚胺培南及美罗培南均耐药,多黏菌素B敏感率为75%,对头孢哌酮/舒巴坦耐药率及中介率各为50%;使用头孢哌酮/舒巴坦治疗,10例好转,2例死亡。结论对于耐碳青酶烯类鲍氏不动杆菌医院获得性肺炎,头孢哌酮/舒巴坦有一定疗效。

OBJECTIVE To summarize the clinical characteristics of pneumonia caused by carbapenem-resistant Acinetobacter baumannii and explore therapeutical methods. METHODS The clinical data and antibiotic susceptibility tests of 12 cases were reviewed. RESULTS Pneumonia caused by carbapenem-resistant A. baumannii occurred in patients complicated with other diseases. X-rays showed bilateral infiltration in lungs. Totally 12 strains were resistant to both imipenem and meropenem. The susceptibility rate to polymyxin B was 75%. The resistant rate to cefoperazone/sulbactam was 50%, however, the intermediate rate was also 50%. Patients were treated with cefoperazone/sulbactam. Ten patients were improved and still in survival,2 patients were died within one month. CONCLUSIONS Cefoperazone/sulbactam shows therapeutical effect in the treatment for hospitalacquired pneumonia caused by carbapenem-resistant A. baumannii.
