无水乙醇对肝脏射频消融增效作用的实验研究

摘要 目的:研究不同治疗方法下集束型cool-tip水冷射频针在离体牛肝上的消融范围,为临床制定消融方案和改善局部疗效提供实验依据。方法:使用美国cool-tip射频治疗仪和集束型中空冷循环消融电极,对离体牛肝进行消融。实验分为3组,Ⅰ组:射频消融组(对照组);Ⅱ组:联合无水乙醇注射组;Ⅲ组:延长消融时间组。消融结束后沿针道切开牛肝,观测组织凝固性坏死形态和范围。结果:牛肝上射频消融后的凝固性坏死灶多呈类圆形;Ⅱ组和Ⅲ组形成的消融体积较其对照组显著增大(P<0.05)。结论:联合无水乙醇注射和适当延长射频消融时间可以增大射频消融的坏死范围。 Objective:To investigate the influence of cluster cool tip radiofrequency ablation on the necrosis range under different treatment method and provide experimental evidence for clinical ablation program and better local curative effect. Methods: Cluster cool tip radiofrequency electrodes with water cooled shaft were used in fresh ox livers. Three groups were set up, group Ⅰ : radiofrequeney ablation (control group) ; group Ⅱ : combined with ethanol injection; group Ⅲ : prolonged ablation time. Ox livers were cut off along the needle tract and the size and shape of coagulation necrosis zones were observed and measured. Results: The coagulation necrosis zones in the ox livers looked similar to the round; the necrosis volume of group Ⅱ and group Ⅲ are bigger than those of in control group (P〈0.05). Conclusions: Necrosis range of cluster cool tip radiofrcquency ablation could be enlarged by being combined with ethanol injection and being prolonged ablation time.
