丙型肝炎感染的模式及其临床价值

Modes of Hepatitis C Infection and Their Clinical Significance

目的检测丙型肝炎游离核心抗原、抗体及HCV-RNA病毒载量,探讨丙型肝炎的感染模式及临床意义。方法对179份已经确诊的HCV感染血样,采用酶联免疫法检测HCV抗体和HCV核心抗原,荧光定量PCR检测HCV-RNA,观察丙型肝炎的感染模式。结果丙型肝炎的感染至少存在7种模式:Ⅰ型HCVcAg(-),HCVAb(+),HCV-RNA(-);Ⅱ型HCVcAg(+),HCVAb(-),HCV-RNA(+);Ⅲ型HCVcAg(-),HCVAb(-),HCV-RNA(+);Ⅳ型HCVcAg(+),HCVAb(-)HCV-RNA(-);Ⅴ型HCVcAg(-),HCVAb(+),HCV-RNA(+);Ⅵ型HCVcAg(+),HCVAb(+),HCV-RNA(+);Ⅶ型HCVcAg(+),HCVAb(+),HCV-RNA(-)。结论联合检测HCV抗体、HCVcAg、及HCV-RNA,分析丙型肝炎的感染模式,对丙型肝炎病情的诊断和治疗具有重要意义。

Objective To detect the core antigen（cAg）,antibody（Ab） and HCV-RNA of hepatitis C and to assess their clinical significance in hepatitis C infection models.Methods Using enzyme-linked immunosorbent assay（ELISA） and fluorescence quantitative PCR,we detected HCVAb,HCVcAg and HCV-RNA in the 179 blood samples of patients with chronic hepatitis C,and summarized the patterns of hepatitis C infection.Results Hepatitis C infected human beings with at least seven modes,i.e.（1） HCVcAg（-）,HCVAb（＋）,HCV-RNA（-）;（2） HCVcAg（＋）,HCVAb（-）,HCV-RNA（＋）;（3） HCVcAg（-）,HCVAb（-）,HCV-RNA（＋）;（4） HCVcAg（＋）,HCVAb（-）,HCV-RNA（-）;（5） HCVcAg（-）,HCVAb（＋）,HCV-RNA（＋）;（6） HCVcAg（＋）,HCVAb（＋）,HCV-RNA（＋）;（7） HCVcAg（＋）,HCVAb（＋）,HCV-RNA（-）.Conclusion Analysis of hepatitis C infection modes through combined serum HCVAb,HCVcAg and HCV-RNA test has great significance in the diagnosis and treatment of HCV.
