住院患者国际抗微生物药用药评价指标体系的应用

The utilization of use of international evaluation indicator system of antimicrobials in inpatients

目的:评价上海市嘉定区中医医院抗微生物药物的管理和使用现状,以提高医院合理用药水平。方法:针对本院2008年的住院患者,采用回顾性方法,依据国际抗微生物药物使用调研指标进行抽样调研。医院整体情况数据通过管理部门和药库获得,处方行为和患者治疗情况通过系统抽样方法采集。结果:住院期间每张处方中使用抗微生物药物≥1种的患者占所观察住院患者的77.78%;每张处方平均使用抗微生物药物1.67种;每例患者使用抗微生物药物平均费用为597.99元;患者使用抗微生物药物治疗(非预防Ⅰ、Ⅱ类切口感染)的平均天数是5.33 d;98.11%的手术患者接受抗微生物药物预防性治疗;77.27%的肺炎患者按相关指导原则治疗;接受抗微生物药物治疗的患者的平均住院天数为13.85 d;接受抗微生物药物治疗者仅15.48%进行了药敏试验。结论:本院住院患者中抗微生物药物使用比例偏高,应强化《抗菌药物临床应用指导原则》的实施,以提高合理用药水平。

Objective： To investigate the administration and utilization of antimicrohials in Traditional Chinese Medical Hospital of Jiading District,in order to promote the rational use of drugs. Methods.. The international antimicrohials evaluate indicator system was applied to evaluate the utilization of antimicrobials in inpatients retrospectively. The global information was gained from administration department and drug storage house of the hospital. The prescription and treatment data were sampled from hospital information system. Results：The inpatients treated with antimicrobials accounted for 77. 78% in all inpatients investigated. The average number of kinds of antimierobials was 1.67 per prescription. The average cost of antimicrohials was 597.99 Yuan per patient. The average duration of the treatment with antimicrohials（not for prevention of Ⅰ,Ⅱ category incision infection） was 5.33 d. About 98.11% of the surgical inpatients treated with antimicrobials for infection prevention and 77.27% of the inpatients with pneumonia received the standard treatment according to the guidelines. The average time of hospitalstay of inpatients treated with antimicrobials was 13.85 d and only 15.48%of them received the drug susceptibility test. Conclusion：The rate of use of antimicrobials in inpatients was high. Measures should be taken to strengthen the implementation of Guidelines for Clinical Use of Antibacteriais in order to promote the rational use of drugs.
