心理护理干预对外伤后脊髓损伤患者的影响

Impact of Psychological Nursing Interventions on Negative Emotion and Quality of Life in Posttraumatic Spinal Cord Injury

目的探讨心理护理干预对外伤后脊髓损伤患者负性情绪和生活质量的影响。方法将164例外伤后脊髓损伤患者随机分为观察组和对照组各82例,对照组接受常规治疗和护理,观察组在此基础上实施为期1个月包括支持性心理护理、认知行为治疗、放松疗法和家属辅助心理疏导在内的综合心理护理干预。比较两组患者干预前后抑郁焦虑评分、脊髓独立功能评分数和生活质量评分。结果两组患者干预后的抑郁焦虑评分明显下降,脊髓独立功能评分明显提高,且观察组的抑郁焦虑评分、脊髓独立功能评分和生活质量、生理领域、心理领域和社会关系因子评分的改善明显优于对照组,差异有统计学意义(t=6.197,5.382,6.473,2.559,2.700,4.621,P<0.01或P<0.05)。结论心理护理干预可改善外伤后脊髓损伤患者的负性情绪和躯体功能状态,并可提高其生活质量。

Objective To investigate the effect of nursing interventions on negative emotion and quality of life in patients with posttraumatic spinal cord injury（SCI）. Methods A total of 164 SCI patients were randomly assigned to the control and observation groups,each consisting of 82 cases. The control group received routine treatment and nursing care ,while the observation group received integrated nursing intervention including supportive mental nursing,cognitive behavioral therapy and relaxation treatment and family-assisted mental guidance for 1 month in addition. A comparison was made in both groups in terms of HRSD,HAMA,SCIM and quality of life scores before and lmonth after intervention. Results The scores of HRSD and HAMA were markedly reduced and the SCIM score was significantly elevated in both groups after intervention, and the scores of HRSD, HAMA, SCIM and quality of life such as physiological domain, psychological domain and social relation domain after intervention were considerably improved in the observation group compared with the control group, showing significant differences between two groups, （t=6.197,5.382,6.473,2.559,2.700,4.621 respectively P〈0.01 或 P〈0.05）. Conclusion The integrated mental nursing intervention can improve the negative emotion and physical function in SCI patients, increasing their quality of life.
