自身抗体检测在系统性红斑狼疮诊断中的价值

Value of autoantibodies detection on diagnosis of SLE

目的评价血清Smith抗体(抗-Sm)、抗核小体抗体(AnuA)、组蛋白抗体(AHA)、双链DNA抗体(抗-ds-DNA)、脱氧核糖核蛋白抗体(抗-DNP)、抗核抗体(ANA)联合检测在系统性红斑狼疮(SLE)诊断中的临床应用价值。方法抗-ds-DNA及ANA采用间接免疫荧光法检测,抗-Sm采用免疫印迹法检测,AnuA和AHA采用酶联免疫吸附试验检测,抗-DNP采用胶乳凝集法检测。结果56例SLE患者抗-ds-DNA、AnuA、AHA、抗-DNP、抗-Sm、ANA的阳性率分别为48%、46%、42%、51%、32%和100%。6项自身抗体联合检测的阳性率为100%,诊断效率为100%。结论自身抗体联合检测大大提高了SLE的敏感性和诊断效率,可避免因单项检测出现的误诊和漏诊情况,对SLE的诊断和治疗有重要意义。

Objective To appraise the eIinical value of serum anti-Sm antibody,anti nucleosomes antibody,anti-histone antibody, anti double-stranded DNA anHbody （dS DNA）, anti-DNP antibody, anti-nuclear antibody （ANA） joint detection in the diagnosis of systemic lupus erythematosus. Methods dS-DNA and ANA were detected by indi- rect immunofluoreseent（IIF） assay,anti-Sm antibody by immuno blotting assay, AunA and anti-histonic antibody by enzyme-linked immunosorbent assay,anti-DNP antibody by latex agglutination assay. Results Among the 56 cases, the positive rates of ds-DNA, AnuA,anti histone antibody,anti-DNP antibody,anti-Sin antibody, ANA were 48%, 46% ,42%,51G ,32G, 100% ,respectively. The total positive rate was 100%. The diagnostic efficiency was 100%. Conclusion The joint detection of six indexes greatly enhances the diagnostic sensitivity and efficiency of SLF,can a void misdiagnosis and missed diagnosis caused by single detection assay. The joint detection is important on the diag- nosis and treatment of SLE.
