卒中患者脑微出血的临床及影像学研究 被引量:1

摘要 目的探讨脑卒中患者脑微出血(CMBs)的临床特点及其影像学特征。方法对32例存在CMBs卒中患者的临床资料及影像学特点进行分析。结果32例患者伴陈旧性腔隙性梗死灶(2.1±1.5)个,白质疏松评分(4.9±2.9)分,画钟试验评分(1.7±1.2)分。出血性卒中9例,CMBs灶(5.7±4.8)个,缺血性卒中23例,CMBs(2.9±2.4)个,组间CMBs数目比较差并有统计学意义(U=55.000,P=0.037)。共检出CMBs118.0个,基底节丘脑区最多为54.0个,其次是皮质-皮质下区44.0个,幕下区最少为20.0个,区域分布差异无统计学意义(χ^2=4.289,P=0.168)。CMBs数目与腔隙性梗死灶数目及白质疏松程度正相关(r=0.276,P=0.014;r=0.431,P〈0.001)。cMBs在磁共振梯度回波T2加权成像上表现为直径2—10mm的圆形或椭圆形信号丢失区域。结论CMBs患者多伴高龄、高血压及执行功能的下降,与腔隙性梗死及白质疏松程度有相关性;GE-T2WI能较好地检出CMBs的存在,有助于评估卒中患者的病变特征及指导下一步治疗。
