中西医结合治疗周围性面神经麻痹疗效观察

Observations on the Efficacy of the Combined Treatment of Peripheral Facial Paralysis with Traditional Chinese Medicine and Western Medicine

目的观察中西医结合法治疗周围性面瘫的临床疗效。方法从门诊确诊的面瘫患者中随机抽取95例,按治疗方法的不同将其分为中西结合治疗组38例、中药组28例和西药组29例。结果中西医结合组面瘫治愈率65.8%,明显高于中药组的28.6%(P<0.01)和西药组的31.0%(P<0.05),而中药组与西药组之间无明显差别;三组有效率分别为94.7%,85.7%,86.2%,其差别无统计学意义(χ2=1.847,P>0.05);依Portmann简易评分法,中西结合组面肌功能评分为17.5263±5.3864,明显高于中药组的13.4286±5.7053(P<0.01)和西药组的13.6897±5.8009(P<0.01),其面肌功能改善率86.92%±28.33%亦明显高于中药组的63.89%±30.11%(P<0.01)和西药组的65.31%±30.82%(P<0.01)。结论中西医结合法治疗周围性面瘫在症状改善及痊愈率上远优于单一的中药疗法或西药疗法,从而可有效减轻面瘫后遗症程度及减少后遗症的发生率。

Objective To investigate the clinical efficacy of the combined treatment of peripheral facial paralysis with traditional Chinese medicine and Western medicine. Methods Ninety-five cases were randomly enrolled from the outpatients diagnosed with facial paralysis and allocated, according to different therapeutic methods, to a combined traditional Chinese and Western medical treatment group of 38 cases, a Chinese herbal medicine group of 28 cases and a Western medicine group of 29 cases. Results The cure rate was 65.8% in the combined traditional Chinese and Western medical treatment group, which was significantly higher than 28.6% in the Chinese herbal medicine group （P〈0.01） and 31.0% in the Western medicine group （P〈0.05）. There was no statistically significant difference between the Chinese herbal medicine group and the Western medicine group. The efficacy rates in the three groups were 94.7%, 85.7% and 86.2%, respectively. There was no statistically significant difference among them （X^2=1.847, P〉0.05）. The Simplified Portman Assessment showed that the facial muscle function score was 17.5263±5.3864 in the combined traditional Chinese and Western medical treatment group, which was significantly higher than 13.4286±5.7053 in the Chinese herbal medicine group （P〈0.01） and 13.6897±5.8009 in the Western medicine group （P〈0.01）. The rate of improvement in facial muscle function was 86.92%_28.33% in the combined traditional Chinese and Western medical treatment group, which was also significantly higher than 63.89%±30.11% in the Chinese herbal medicine group （P〈0.01） and 65.31%±30.82% in the Western medicine group （P〈0.01）. Conclusion A combined treatment with traditional Chinese medicine and Western medicine is much superior to simple Chinese herbal treatment or Western drug treatment in improving the symptoms of peripheral facial paralysis and in the cure rate. It can effectively lighten the sequela of facial paralysis and decrease its incidence.
