摘要
目的了解我国艾滋病高发地区艾滋病感染孕产妇所生婴儿的死亡状况,为降低艾滋病感染孕产妇所生婴儿死亡水平提供参考依据。方法针对云南、新疆、广西和河南4个省(自治区)7个县(市、区)艾滋病感染孕产妇所生婴儿的死亡状况,对死亡婴儿的父母或其他看护人进行调查,通过查阅儿童死亡前在医疗机构就诊的住院病历、婴儿死亡登记卡、婴儿预防艾滋病母婴传播随访调查表和实验室检测结果等途径调查核实艾滋病感染患儿的死亡状况。结果不同研究地区艾滋病感染孕产妇所生婴儿死亡水平不同,其中广西地区高达164.87/1000人年;艾滋病感染婴儿与非艾滋病感染婴儿的死亡水平之比为5.65;艾滋病感染孕产妇所生婴儿疑似艾滋病死亡占到40.00%(18/45),其次为早产低出生体重。结论我国艾滋病高发地区艾滋病感染孕产妇所生婴儿死亡水平较高,需要进一步分析探讨其原因。
Objective To investigate death situation of infants born by HIV-infected mothers in HIV highly epidemic regions in our country, so as to provide reference basis for reducing mortality rate of infants born by those HIV-infected mothers. Methods In Yunnan province, Xinjiang autonomous region, Guangxi Zhuang nationality autonomous region and Henan province, mortality of infants born by HIV-infected mothers were investigated and verified with a Death Cause Deducing Scale by questioning the parents or other caregivers of the dead infants and reviewing medical recordes of dead infants in medical institutions, death registration cards of infants, infants' PMTCT follow-up questionnaire and laboratory results. Results The mortality rate of infant born by HIV-infected mothers in different regions is varied, of which, the highest mortality rate ( 164.87 per 1000 infant-year) was seen in Guangxi Zhuang nationality autonomous region. The ratio of mortality rate of HIV-infected infants over that of HIV-uninfected infants born by HIV-infected mothers was 5.65 : 1. The death of infants born by HIV-infected mothers who died of suspected HIV infection accounted for 40% ( 18/45 ) of the total death, and premature low birth weight was the second cause of death of the infant. Conclusion Mortality rate of infants born by HIV-infected mothers in HIV highly epidemic regions in our country is in high level. So, it is necessary to investigate and analyse cause of death of infants those born by HIV- infected mothers further.
