腹腔镜卵巢囊肿剔除术对卵巢功能的近期影响

Short-term effect of laparoscopic oophorocystectomy on ovarian function

目的研究腹腔镜良性卵巢囊肿剔除术对卵巢功能的近期影响。方法选择卵巢囊肿拟行囊肿剔除术57例患者，随机分为对照组和腹腔镜组。所有患者分别于术前、术后1个月、术后3个月测定血雌二醇、孕酮、睾酮、卵泡刺激素、黄体生成激素的水平，并进行术后随访。结果所有病例在术后1个月均出现雌二醇下降及卵泡刺激素和黄体生成激素增高，两组分别与术前比较均有显著性差异（t分别为0．4863、0．7411、0．3807、0．7926、0．9537、1．2195，均P〈0．05），但两组之间术后雌二醇、卵泡刺激素、黄体生成激素比较无显著性差异（t分别为4．1141、2．1827、2．2469，均P〉0．05）。术后3个月对照组患者的月经异常例数稍多，但两组间比较无显著性差异（Х^2=0．5304，均P〉0．05）。结论腹腔镜良性卵巢囊肿剔除术未加重卵巢近期功能的损害。

Objective To investigate short-term effect of laparoscopic oophorocystectomy on ovarian function. Methods 57 patients with ovrian cysts were randomly divided into two groups： conventional abdomen-open oophorocystectomy group （control group） and laparoscopic oophorocystectomy group. The serum levels of estradiol （ E2 ）, progesterone（ P）, testosterone（ T）, follicle-stimulating hormone （ FSH）, luteinizing hormone （LH） of all patients were detected before operation and at 1 month and 3 months after operation respectively and all of them received postoperative follow up. Results At 1 month after opertation, the serum level of E2 of all patients was lowered and their serum levels of FSH and LH were increased as compared with that before opreation, there were significant differences （ t = 0. 4863, 0. 7411,0. 3807,0. 7926, 0.9537 and 1. 2195 respectively, all P 〈 0.05 ）. But in postoperative serum levels of E2, FSH and LH, there were no significant differences between the two groups （ t = 4. 1141, 2. 1827, 2. 2469 respectively, all P 〉 0.05 ）. At 3 months after operation, proportion of the patients whose menstruation was abnormal in the control group was slightly higher than the laparoscopic oophorocystectomy group, but the difference was not significant （Х^2 = 0. 5304, P 〉 0. 05 ）. Conclusion The laparoscopic oophorocystectomy does not exacerbate damage of ovarian function in short-term after operation.
