我院门诊处方规范性检查分析

Data analysis of the standardization of outpatient prescriptions

目的了解我院门诊处方的规范化水平,分析药品应用的合理性。方法随机抽查2007年5月-2009年10月门诊处方6000张,按照相关规定,对处方的规范性和用药的合理性进行分析评价。结果不合格处方有1440张,占所抽查处方的24.00%,其中处方格式或书写不规范的占54.65%,用药不合理占45.35%。用药不合理中,无适应证用药、药品用法用量不当及选药不当是最突出的问题。结论我院亟待进一步强化门诊处方的监督管理,规范医生处方行为。

Objective To assess the standardization of outpatient prescriptions in Shuguang Hospital and to evaluate their rationality. Methods A total 6000 outpatient prescriptions were randomly selected for the analysis based on relevant guideline and regulation. Results Total 1440 （24.00%） outpatient prescriptions were found to be irrational and 54.6% prescription were non-standard format or writing. The irrational drug use was of 45.35%. Within the irrational prescriptions the major problems were unconformity between drug use and clinical diagnosis, incorrect dosage, improper use and irrational choice of drugs. Conclusion It is important to strengthen the management of outpatient prescriptions to improve their rationality.
