“胆病从肝论治”在防治胆结石实验研究中的应用

The Application of'Biliary Diseases Treated by Differentiating Syndromes of the Liver' to the Experimental Research on Prevention and Treatment of Gall Stones

目的:以"胆病从肝论治"为原则研究大黄灵仙胶囊防治胆结石的作用机制。方法:使用致石饲料诱发方法建立豚鼠胆囊结石模型,治疗组造模期间每日予大黄灵仙胶囊(制作成悬液)灌胃治疗,连续8周。观察比较治疗组和模型组豚鼠胆囊结石形成率,检测各组豚鼠肝脏病理和超微结构变化情况以及荧光定量PCR检测肝组织胆固醇、胆汁酸代谢相关酶的表达量。结果:模型组和治疗组豚鼠胆囊结石形成率分别是100%和50%,差异有统计学意义(P<0.01)。治疗组豚鼠肝组织病变较模型组明显减轻,胆固醇7a-羟化酶、胆汁酸盐输出泵基因表达分别是模型组的2.08倍和2.57倍(P<0.01)。结论:大黄灵仙胶囊具有较好的防治结石功效,作用机制与其保护肝组织以及在转录水平上调胆固醇、胆汁酸代谢相关酶的表达有关。"胆病从肝论治"在胆结石防治实验研究中具有重要的指导价值。

Objective：To investigate the mechanism of Dahuanglingxian Capsules in preventing and treating gall - stone diseases based on the principle of ＂Biliary Diseases Treated by Differentiating Syndromes of the Liver＂. Methods： The gall -stone models of guinea pigs were induced by being fed with lithogenic diet, at the same time, the treated group were fed with lavage of Dahuanglingxian Capsules （ made into suspension fluid ） once per day. Gall - stones formation ratio of the model group and the treated group were statistlled 8 weeks later. The routine pathology and uhrastructure of liver were detected. Genes expressions of cholesterol and bile acid metabolism related enzymes were quantitatively analyzed by real time PCR. Results： Gallstones formation ratio of the model group and the treated group were 100% and 50% respectively. Gallstones formation ratio of the treated group was significantly decreased compared with the model group （P 〈0. Ol ）. Liver routine pathology and ultrastructure of the treated group were obvious better than that of the model group. The CYP7AlmRNA and BSEPmRNA expressions of the treated group were 2.08 times and 2.57 times of those of the model group respectively （P 〈 0. 01 ）. Conclusions： Dahuanglingxian Capsules have a good effect on pre venting gallstones formation, the mechanism of which was associated with its liver protection and up - regulation of the expressions of cholesterol and the related enzymes of bile acid metabolism on the transcription level. The principle of ＂biliary diseases treated by differentiating syndromes of the liver＂ embodies the value of guidance in the experimental research on prevention and treatment of gall stones.
