IgA肾病与过敏性紫癜性肾炎临床病理比较

Clinical and pathological comparison of IgA Pephropathy and Henoch-chonlein purpura nophritis

比较了２８例ＩｇＡ肾病（ＩｇＡＮ）和１９例过敏性紫癜性肾炎（ＨＳＰＮ）的临床和病理资料，结果表明：Ｉ－ｇＡＮ与ＨＳＰＮ有许多相似之处，二者血尿发生率、男女发病率和高血压发生率相似；肾脏病理Ⅱ、Ⅲ、Ⅴ和Ⅵ级发生率相似；免疫病理ＩｇＡＧ型和ＩｇＡＭ型的发生率亦相似。但二者之间亦有不同之处，ＩｇＡＮ发病年龄、肉眼血尿发生率、血清ＩｇＡ升高率高于ＨＳＰＮ；肾病综合征（ＮＳ）发生率低于ＨＳＰＮ；且ＩｇＡＮⅣ级病变和免疫荧光ＩｇＡＧＭ型的发生率高于ＨＳＰＮ。上述差别均有统计学意义。此外，ＩｇＡＮ肾功能异常发生率高于ＨＳＰＮ，单纯ＩｇＡ沉积型低于ＨＳＰＮ，但无统计学意义。

In this study we had compared the clinical and pathological materials of 28 IgAN and 19 HSPN patents.The results revealed that there were many similarties between IgAn and HSPN.They shared similar occrurrence rates of hematurias、male/remale and hypertention.There were also similar occurrence rates of either biospy grades Ⅱ、Ⅲ、Ⅳ、Ⅵ or immunopathologic type IgAG and IgAM between IgAN and HSPN.But there were some difference beetween the tow diseases.The age of IgAN patients was higher than HSPN.The occurrence rates of macroscopic hematuria、biospy grade Ⅳ and immunopathologic type IgAGM were higher,but the occurrence rate of nephrotic syndrome was lower in IgAn as compared with HSPN.(all P<0.05) In addition:IgAN and higher occurrence rate of renal abnormal and lower occurrence rate of immunopathologic type IgA than HSPN,but had no statistical significance.Our materiails provided some clincal evidence for evaluate the relationships between IgAN and HSPN.
