腹腔镜治疗急性结石性胆囊炎145例体会

Experience of laparoscopic cholecystectomy for acute calculous cholecystitis

目的总结腹腔镜胆囊切除术治疗急性结石性胆囊炎的体会。方法对我院145例急性结石性胆囊炎患者采取LC进行治疗。结果平均发作时间82.3h,手术平均时间64min,出血量平均23ml,平均引流量36.3ml。中转开腹4例(占2.7%),发生胆漏2例,膈下感染1例,无手术死亡。患者均痊愈出院,平均住院时间12.96d。结论准确掌握胆囊结石并发急性胆囊炎腹腔镜切除术的适应证,手术是安全可行的。

Objective To summarize the effect of laparoscopic cholecystectomy （LC） for treatment of acute calcu- lous cholecystitis. Methods One hundred and forty - five acute calculous cholecystitis patients underwent LC were reviewed. Results The average onset time of acute calculous cholecystitis was （ 82.3 ± 5 ） hours, the mean operation time was （64 ± 20） minutes and the average blood loss was （ 23 ± 5 ） ml, the average drainaging amount was （ 36.3 ± 12） ml. 4 cases were conversed to open cholecystecomy （2.7%）. The complications were：biliary leak （2 cases） ;infection of diaphragm inferior（ 1 case）. 145 cases were cured, and the average hospitalization time was 12.96 days. Conclusions Laparoscopic cholecystectomy is safe and feasible in the treatment of acute calculous cholecystitis.
