单纯腹痛型过敏性紫癜128例误诊分析

Analysis on Misdiagnosis of 128 Cases with Simple Abdominal Anaphylactoid Purpura

目的:通过对我院128例单纯腹痛型过敏性紫癜的误诊分析,说明在单纯腹痛型过敏性紫癜的诊断中,护士对腹痛全方位的仔细观察是正确诊断这类疾病的重要因素之一。方法:对我院128例单纯腹痛型过敏性紫癜腹痛的发作时间、持续时间、疼痛程度、疼痛部位是否固定、与饮食的关系、是否有牵扯或放射痛、有无伴随症状、是否对镇痛药物敏感等八个方面进行分析,了解单纯腹痛型过敏性紫癜的腹痛与其他疾病的腹痛症状的区别,为确诊这类疾病提供有效的护理观察依据。结果:通过仔细观察,单纯腹痛型过敏性紫癜的腹痛有一定的特殊性和规律性,这类病人的腹痛一般具有发作时间不确定性,持续时间较长,疼痛程度自觉症状较重、体征较轻,疼痛部位根据出血部位而定,与是否进食无关,无放射或牵扯痛,无明显伴随症状,解痉镇痛效果较差。结论:对单纯腹痛型过敏性紫癜的诊断比较困难,详细作好这类腹痛病人的护理观察记录,将大大提高这类疾病的诊断准确率。

Objective： To analyze the misdiagnosis of 128 cases with simple abdominal anaphylaetoid purpura, so as to explain that comprehensive observation on abdominal pain is one of the important factors for the accurate diagnosis of this disease. Method.. To see if they are fixation in attack time, duration time, pain degree and painful area. To analyze the relationship with diet. To observe if they have radicular pain and accompanied symptoms or sensitive to analgesic drugs. To understand the difference of abdominal pain between simple abdominal anaphylactoid purpura and other diseases, so as to provide effective nusing basis for the diagnosis of this disease. Result： The findings shows that the abdominal pain of simple abdominal anaphylactoid purpura is of pe-culiarity and regularity. The pain has long duration time with uncertain time of attacking, heavy subjective symptoms and light physical signs. The painful area is determined by bleeding site. It doesn＇t have radicular Fain and obvious accompanied symptoms. Conclusion： It is difficult to diagnose the simple abdominal anaphylactoid purpura. We should make detailed record of nursing observation, which will greatly improve the accurate rate of diagnosis in this disease.
