黄韧带增生肥厚的影像学研究及临床病理分析 被引量:4

摘要 目的为MR的T1轴位加权像精确定位测量腰椎管狭窄患者增生肥厚黄韧带提供解剖学依据,探讨年龄、椎体节段因素与增生肥厚黄韧带关系及临床病理特征。方法(1)分析我院收治的58例因黄韧带增生肥厚导致腰椎管狭窄产生下腰痛患者的影像学资料,在MR的T1轴位加权像经关节突平面,于L3-4,L4-5和L5S1椎体节段精确测量增生肥厚黄韧带厚度,分析黄韧带厚度与年龄、椎体节段的相关性;(2)在腰椎手术中,收集腰椎管狭窄(LSS)患者不同厚度增生肥厚黄韧带10例,行Masson染色对比,探讨黄韧带增生肥厚的临床特征和病理特征。结果(1)经关节突平面可最佳测量黄韧带厚度值,随患者年龄增大黄韧带厚度增加;(2)L4-5椎体节段黄韧带厚度明显高于其他椎体节段;(3)Masson染色后增生肥厚黄韧带内部表现为弹性纤维减少,胶原表达增多;(4)增生肥厚黄韧带纤维化程度与黄韧带厚度呈显著正相关。结论腰椎管狭窄患者增生肥厚黄韧带病理特征表现为内部弹性纤维减少、胶原增加所致的纤维化。年龄增长导致黄韧带厚度增加、纤维化程度加重,L4-5节段黄韧带厚度明显高于其他脊柱节段。
