胃癌患者手术前后T细胞及其活化抗原表达的变化与临床意义

Alterations in T Cell Subsets and Activation Antigens in Patients with Gastric Cancer before and after Operations and Their Clinical Significances

目的:研究胃癌患者围手术期的免疫状态.方法:应用流式细胞仪检测33例胃癌患者手术前后T细胞表面6种细胞表型,并与良性病变患者进行对比分析.结果:胃癌患者术前CD3^+、CD4^++、CD16^+、CD69^+细胞以及CD4^+/CD8^+细胞比值明显低于对照组.CD8^+细胞显著高于对照组(P<0 01),CD3^+/HLA-DR^+细胞与对照组相同;术后CD8^+细胞降低,CD3^+、CD4^+、CD4^+/CD8^+、CD16^+、CD69^+、CD3^+/HLA-DR^+均升高(PM<0.01).CD3^+/HLA-DR^+的活化T细胞明显高于对照组(P<0.01).结论:胃癌患者术前免疫功能低下,术后细胞免疫功能改善,活化T细胞明显增多.

To evaluate the peri-operative immune states of patients with gastric cancers, the T cell subsets and the activation antigens were determined by flow cytometrical analysis in 33 cases of gastric cancer before and after operations, and the data were compared with those in the controls with benign diseases. It was found that the CD3+, CD4+, CD16+, CD69+ cells and the CD4/CD8 ratio were significantly lower in cancer patients before operations than those in the controls, and the CD8+ cells were much higher in cancer patients. The CD3+, HLA-DR+ cells showed no significant difference between the cancer patients and the controls. Post-operatively, the CD3+ , CD4+, CD16+, CD69+, CD3+, HLA-DR+ cells as well as the CD4/CD8 ration all increasedsignificantly, while the CD8+ cells decreased in cancer patients. Furthermove, the CD3, HLA-DR activated T cells were rernarkablly higher than those of the controls. It concludes from this study that the immune status of patients with gastric cancers was somewhat depressed, and the increase of activated T Cells after operations indicates the improvement of cellular immunity in cancer patients.
