国产食管自膨式支架置入术后并发症的发生及防治探讨

Incidence and Prevention of Complications after Domestic Self-expanding Metallic Stent for Esophageal Stenosis

目的分析国产食管自膨式支架置入术后并发症的发生及处置情况,探讨其防治方法.方法回顾性分析43例接受国产食管自膨式支架置入术后患者并发症的性质、规律及处理过程,并对处置后的疗效进行分析、总结.结果国产食管自膨式支架治疗食道良恶性狭窄病人可明显改善患者吞咽功能.术后所有病人都有不同程度的胸骨后疼痛等并发症;4例反流性食管炎;3例支架移位;3例再狭窄;1例消化道大出血;1例食团阻塞支架;无穿孔和手术死亡等严重并发症.结论食管内支架治疗食管狭窄是一种简单、安全、可靠的方法.支架放置后的并发症经适当治疗可得到有效控制.

Objective To investigate the complications of esophageal stenosis treated with domestic self-expanding metallic stent and evaluate the treatment and prevention of complications.Methods The complications of 43 patients with esophageal stenosis treated with domestic self-expanding stent were retrospectively analyzed.Results 43 patients had made significant improvement in dysphagia.Thoracic pain happened in all cases,reflux esophagitis in 4 cases,stent migration in 3 cases,restenosis in 3 cases,massive esophageal hemorrhage in 1 case,food obstruction in 1 case.Serious complications such as perforation and stent-induced death did not occur.Conclusion Stents placement is a safe,simple and effective method to treat the esophageal stenosis.Complications after metallic stent placement for esophageal stenosis are common,but can be treated successfully in most cases.
