23例小肠间质瘤外科治疗的临床分析

Surgical treatment of 23 cases of small intestinal stromal tumor

目的探讨小肠间质瘤的诊断及外科治疗。方法回顾性分析该院自2004年1月～2008年12月行手术治疗的23例小肠间质瘤患者。结果中位年龄为57.4岁,主要症状为腹痛、下消化道出血和腹部肿块。肿瘤发生于十二指肠4例,空肠10例,回肠9例。所用患者均行手术治疗。经病理及免疫组织化学证实。结论小肠间质瘤缺乏典型的症状、体征及有效的检查方法。CT及胶囊内镜是有益的检查手段。最终诊断依赖于病理学。最有效的治疗方法是根治性手术切除。复发病例仍可再手术。

【Objective】To investigate the experience in the surgical treatment of small intestine stromal tumor. 【Methods】23 patients were retrospectively reviewed with small intestine stromal tumor in our hospital from Jan. 2004 to Dec. 2008.【Results】The average age of patients in this group was 57.4. The major clinical signs included： abdomen pain, lower gastrointestinal hemorrhage, abdomen tumor. In this group, 4 cases occurred in duodenum, 10 cases in jejunum, and 9 caeses in ileum. All cases were operated. The specimens were confirmed by pathology and immunohistochemistry. [ Conclusions ] Small intestine stromal tumor lacks special clinical symptom, sign and effective examination methods, CT or endoscope collocystis is helpful to diagnosis to a certain. Last diagnosis depends on pathologic examination. The most effective therapy was operation removal of the tumors. Cases with recurrence should be reoperated.
