新生儿缺氧缺血性脑病中TNF-α变化的研究 被引量:2

Investigation on the changes of TNF-α in neonates with hypoxic-ischemic encephalopathy

摘要 目的:探讨新生儿缺氧缺血性脑病患儿外周血TNF-α动态变化及临床意义。方法:采用ELISA法分别于生后第3天与第7天对50例HIE患儿及23例正常新生儿血清TNF-α水平进行动态测定。结果:HIE组急性期不同程度(轻、中、重度)HIE患儿血清TNF-α水平与对照组比较,差异具有统计学意义,轻度HIE组血清TNF-a水平均明显高于对照组(P<0.05),中、重度HIE组血清TNF-α水平均明显高于对照组(P<0.01),且重度组又明显高于轻、中度组(P<0.01),恢复期轻、中、重度HIE 3组患儿血清TNF-a水平与对照组无统计学差异(P>0.05)。结论:TNF-α参与HIE脑损伤的病理生理过程,通过动态观察HIE患儿血清中TNF-α变化,对了解HIE脑损伤程度及判断预后可能具有重要价值。 Objective: To explore the dynamic changes and clinical significance of TNF - α in neonates with hypoxic - ischemic encephalopathy (HIE) . Methods: The levels of TNF -α in serum were detected by enzyme -linked immunosorbent assay (ELISA) on the third day and seventh day after birth in 50 neonates with HIE and 23 neonates without HIE. Results: There was significant difference in TNF - α level between acute stage of HIE group and control group, the TNF - αlevels in mild, moderate and severe HIE group were significantly higher than that in control group ( P 〈 0. 05 ), the TNF - α level in severe HIE group was significantly higher than those in mild and moderate HIE group (P 〈0. 01 ), there was no significant difference in TNF -α level between HIE group in recovery stage and control group (P 〉 0. 05 ) . Conclusion : TNF - α may participate in the pathophysiological process of brain injury induced by HIE. Observation of dynamic changes of TNF - α in neonates with HIE may plays an important role in evaluation of brain injury degrees and judging prognosis of HIE.
