摘要 目的探讨老年慢性阻塞性肺疾病(COPD)合并2型糖尿病(DM2)患者免疫功能状况及临床意义。方法选取64例老年COPD合并DM2住院患者为研究组、老年单纯COPD组35例,老年健康组30例为对照。检测患者外周血IgA、IgG、CD3+、CD4+、CD8+及CD4+/CD8+水平,并进行分析。结果老年COPD并DM2组IgA、IgG、CD4+水平均低于老年单纯COPD组和老年健康组,CD8+水平高于老年单纯COPD组和老年健康组,CD4+/CD8+倒置,差异均有统计学意义(P<0.05),而CD3+水平在各组之间无差异。结论老年COPD并DM2患者存在免疫功能低下,给予增强免疫治疗对提高临床疗效可能有意义。 Objective To investigate the immunoloical function and significance in elderly patients with chronic obstructive pulmonary disease (COPD) complicated type II diabetes (DM2) . Methods A randomized, prospective clinical trial was held, and 64 elderly patients with COPD complicated DM2 were recruited into the study, 35 elderly patients with simple COPD and 30 healthy individuals served as the elderly control. Levels of IgA and IgG were determined with rate nephelometry method, the CD3^+, CD4^+ , CD8 ^+ , CD4 ^+/CD8 ^+ in peripheral blood were measured by flow cytometry. Then the results were analyzed. Results The aged COPD complicated DM2 group had a more decreased IgA, IgG^+ CD4 ^+ level, CD4^+/CD8^+ ratio and more increased CD8 + level, compaed with aged simple COPD control and aged healthy control (P 〈 0. 05) . Conclusion Elderly patients with COPD complicated DM2 have a bad immune function. The immune treatment can help to enhance the immune response and improve clinical effect for the patients.
