睾丸非霍奇金淋巴瘤22例临床分析

Clinical Analysis of Non-Hodgkin's Lymphomas of Testis

〔目的〕回顾性分析星丸非霍奇金淋巴瘤（NHL）的临床表现、治疗效果及各种因素对预后的影响，并对术后放、化疗的作用，提出见解。［方法］睾丸非霍奇金淋巴瘤22例，全部病例均先作根治性切除术、术后病理证实、分类。术后单纯放疗4例，化、放疗结合18例。［结果］以寿命表法计算生存结果1、3、5年生存率分别为72．7％、50．0％和39．9％。［结论］本病不论病期早、晚术后均应予全身联合化疗。放疗仅作区域性引流区病灶的控制。根治术后采取化疗—放疗—化疗是可取的治疗模式。

[Purpose]To review the clinical manifestation, curative effectand the prognostic factors of Non - Hodgkin's lymphoma of estis retrospectiely. [Methods] From 1979 to 1994, 22 cases of Non- Hodgkin'S Lymphoma of the testis have been treated in iyr bospital All patients were treated by orchiectomy, 4 cases were followed by pure radiotherapy, 18 cases were followed by chemotherapy combined with radiotherapy. [Results] The survival rates af 1,3 and 5 years were 72.7%',50.5% and 39.9%, respectively [Conclusion] Non -Hodgkin's lymphoma of the testis spreads rapidly and extensively but it was concidered to be radiosenstive and chemosensitive. The tendency of tesicular Non -Hodgkin's lymphona forearly systematic metastasis suggests need for multiagent chemotherapy in initial management. Radiotherapy following orchietomy can only control regional lesions.Chemotherapy - radiotherapy - chemotherapyfollwing orchiectomy is the optimal treatement modality.
