心肌肌钙蛋白T检测对急性心肌梗死的诊断价值

Cardiac Troponin T Test for the Diagnosis of Acute Myocardial Infarction

目的探讨心肌肌钙蛋白T检测对急性心肌梗死的诊断价值。方法对80例AMI患者和60例CHF患者同时分别于4h、12h、24h、48h、第l0天采静脉血1次,立即进检,进行肌钙蛋白T(cTnT),心肌酶学的测定如肌酸激酶同工酶(CK-MB)和CK。结果AMI组和CHF的cTnT检测阳性率的比较:AMI组80例患者中有76例cTnT检测为阳性,阳性率为95.0%;CHF组60例患者中有15例阳性,阳性率为25.0%。两组比较差异有显著性差异(P<0.01)。AMI组和CHF组各项指标阳性率的比较:在发病后24小时内检测cTnT、CK-MB和CK,结果80例AMI患者中,cTnT阳性60例,阳性率76%;CK-MB阳性34例,阳性率42%;CK阳性36例,阳性率45%。在60例CHF中,cTnT阳性5例,阳性率9%;CK-MB未检出阳性,阳性率0%;CK阳性1例,阳性率2%。结论肌钙蛋白T灵敏检测法可用定性的方法测抗凝血中cTnT的浓度变化,操作简单、快速,可及时获得结果,有利于AMI患者的早期诊断。

Objective To investigate the detection of cardiac troponin T in acute myocardial infarction diagnosis. CHF group, 60 patients, 15 were positive, the positive rate was 25.0%. Difference between the two groups was significant difference （P 〈0.01）. AMI group and CHF group comparison of positive rates of the indicators： within 24 hours after the onset of testing cTnT, CK-MB and CK, the results of 80 cases of AMI patients, cTnT-positive 60 cases, the positive rate of 76%; CK-MB positive for 34 cases, the positive rate of 42%; CK-positive 36 cases, the positive rate of 45%. In the 60 cases of CHF in, cTnT-positive in 5 cases, the positive rate of 9%; CK-MB were not detected positive, the positive rate of 0%; CK-positive in 1 case, the positive rate of 2%. Conclusions Troponin T sensitive detection method for the characterization measurements can be used anti-clotting changes in the concentration of cTnT, simple operation, rapid, timely access to results, and this is conducive to the early diagnosis of patients with AMI.
