原位杂交检测宫颈活检组织HPV感染的临床意义 被引量:3

Clinical Significance of Detection HPV Infection in Cervical Biopsy Tissue by Using in Situ Hybridization

摘要 目的探讨原位杂交(ISH)检测宫颈活检组织中人类乳头瘤病毒(HPV)感染对宫颈癌早期诊断的临床意义。方法对128例宫颈活检的石蜡标本,分别行常规检测和ISH-HPV检测。结果①二种方法检测慢性宫颈炎、宫颈上皮内瘤变(CIN)、宫颈鳞状细胞癌(SCC)HPV感染检出率分别为2.5%与8.9%、37.5%与55.0%和66.6%与90.0%,检出率SCC>CIN>慢性宫颈炎,ISH法明显优于常规检测法(χ2=7.5625,P<0.01)。②在所检测的128例宫颈活检组织标本中,21~30岁患者的HPV感染率明显高于其它年龄组。结论慢性宫颈炎、CIN、宫颈鳞状细胞癌与HPV6/11/16/18/31/33密切相关,随着病变程度的加重,HPV感染的检出率明显升高。ISH方法对早期宫颈HPV感染患者在分子病理学水平作出准确、特异、快速的诊断,同时具有细胞阳性定位功能。通过HPV感染的检测,可以积极预警年轻人群中宫颈癌的发生。 Objective To discuss the clinic significance of HPV detection in the cervical biopsy tissue by in situ hybridization(ISH) in the early diagnosis of cervical cancer.Methods ISH and general technique was utilized to detect BPV in 128 paraffin sample of cervical tissue.Results ①HPV infection in the chronic cervical inflammation,cervical intraepithelial neoplasia(CIN) and cervical squamous cell cancer(SCC) were detected by ISH and general technique methods,and the positive rate was 2.5% and 8.9%,37.5% and 55.0%,66.6% and 90.0% respectively.There was obvious difference between ISH and general technique methods(χ2=7.5625,P0.01).The positive rate was rise evidently SCCCINchronic cervical inflammation.②In all 128 of cervical biopsy tissue,the HPV infective rate in patients of 21 to 30 years old was obviously higher than that in other crowds.Conclusion HPV 6/11/16/18/31/33 infection is closely related to CIN,SCC and chronic cervical inflammation;and the HPV infection positive rate rises with the aggravation of the disease.The situ hybridization technique is dependable,specific and expeditious in the molecular pathology diagnosis of the early HPV infection of cervix.It also has the function of targeting positive cells.HPV detecting can prevent the occurrence of cervical cancer in younger crowd.
