摘要
为了进一步降低机械通气新生儿下呼吸道细菌感染,提高机械通气患儿的存活率。本文分析了我院1985~1996年NICU内应用呼吸器治疗158例患儿并发下呼吸道细菌感染情况,按1985~1989年(甲组)57例、1990~1991(C组)36例、1992~1996年(丙组)65例三阶段进行分析。拟定预防感染措施是:(1)严格预防呼吸治疗装置的污染而引起的感染;(2)气管内冲洗吸痰严格遵守无菌操作,呼吸道管道、接头、湿化加热器内蒸馏水每天更换,消毒;(3)室内空气消毒与保洁;(4)抗生素的应用和支持疗法。结果使下呼吸道分泌物培养由原来的100%(1,3)。降至乙组的47.2%至丙组的35.38%。胸片示肺部感染加重由甲组的46.65%降至乙组的22.2%丙组的9.23%。
To reduce furture the bacterial infection in low respiratory tract of neonatals with machinery ventilation. The stalenese in 158 cases of neonatal admited to NICU during 1985 to 1996 were analysed. All were therapied with machinery ventilation and complicated by the infection of low respiratory tract. There were four kinds of methods which prevented infection in it: (1) Prevent strictly the infection which resulted from the contamination of therapeutic installation. (2)Observed strictly asepsis when rinsed and drawed sputum in trachea and replaced of sterilized tube into respiratory tract, joint and distilled water in the humidheat apparatus every day. (3) sterilized air in NICU; (4)Maked use of antibiotics and give support therapy. The results showed that the positive rates of seeretion culture in respiratory tract decrease from 100 percent to 47. 2 percent in second-group and 35. 38 percent in third-group. X-ray bilm on chest indicated that the aggravation of lung infection decrease from 46. 65 percent in first-group to 22. 2 percent in second-group and 9. 23 percent in third group.
