鼻腔鼻窦内翻性乳头状瘤76例诊治体会

Management of 76 patients with nasal inverted papilloma

目的：比较不同术式治疗鼻腔及鼻窦内翻性乳头状瘤的疗效。方法：参考Krouse的鼻腔鼻窦内翻性乳头状瘤临床分期标准对76例鼻腔鼻窦内翻性乳头状瘤患者进行分期，根据分期行单纯鼻内镜手术、鼻内镜联合柯-陆进路手术、鼻侧切开术。术后随访6～36个月，比较复发率。结果：临床分期相同的患者三种术式疗效无显著性差异（P〉0．05）。总复发率11．84％（9／76）。结论：应充分认识该疾病，完善术前检奁，根据Krouse分期选择合适的术式。术后复发与病变范围和有密切关系。

Objective To compare the therapeutic effect of different procedures on nasal inverted papilloma （N/P）. Method Total 76 cases of NIP were analyzed and divided into 4 different phases based on the Krouse criterion. The cases were treated by three kinds of surgical procedures：lateral rhinotomy, single endoscopic surgery, and endoscopic plus Caldwell -Lue surgery. All the patients were followed up for 6 - 36 months after operation. Results There was no obvious difference on the therapeutic effect of three kinds of surgical procedures in same stage （P 〉 0. 05 ）. The total recurrence rate was 11.84% （9/76）. Conclusion The well understanding of NIP,the detailed examination and selecting the appropriate surgical procedures according to Krouse classification are key to reduce the reeurrence rate of NIP. The recurrence rate is closely related to the extent of disease.
