非创伤性血液丢失对老年髋部骨折围手术期的影响

Effects and management of nontraumatic Hemorrhage on perioperation of hip fracture in elderly patients

目的探讨非创伤性血液丢失对老年髋部骨折围手术期的影响。方法选取我院93例60岁及以上的老年髋部骨折患者资料进行回顾性分析,探讨非创伤性血液丢失与围手术期并发症的关系。结果围手术期并发症中,以下肢深静脉炎发生率最高,其后依次是贫血、应激性障碍、水电解质紊乱、应激性溃疡、下肢神静脉血栓(DVT)、心绞痛及肺栓塞等,其中大多与非创伤性血液丢失有关,而及时补充血容量可使各种并发症的发生率降低。结论老年髋部骨折围手术期血容量的及时足量补充,是减少或避免非创伤性血液丢失并有效阻断继发病变的重要措施,也是手术成功的保障。

Objective It is to approach the influence of nontraumatic blood loss on the peroperative period of hip fracture in elderly patients. Methods 93 elderly patients with hip fracture aged 60 years and over were chosen and reviewed, the rela-tionship between nontraumatic blood loss and complication on the peroperative period was observed. Results The incidence of lower limb phlebitis deep was the highest in all perioperative complications, followed successively by anemia, stress cognition disorder, hydroelectrolytic disorder, stress ulcer, deep venous thrombosis （DVT）, angina, pulmonary thromboembolism （PTE） and so on. This study indicated that most complications were correlated with nontraumatic blood loss, more over, supplementary of blood volume in time could decrease the incidence of complications. Conclusion Enough supplementary of blood volume in time during the peroperative period of hip fracture in elderly patients is important to decrease or prevent nontraumatic blood loss and interrupt secondary reaction effectively, it is also the safeguard key to the operation.
