纤维支气管镜下支气管置入单向阀治疗COPD疗效观察

摘要 目的探讨支气管堵塞肺减容术(bronchoscopic lung volume reduction,BLVR)对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)的疗效。方法选择4例COPD患者,于单向阀置入前、单向阀置入后对通气、血流动力学及血气分析结果进行比较、评价。结果 BLVR后,1 s用力呼气量平均增加0.40 L;用力肺活量平均增加0.31 L;6 min步行距离平均增加了 119 m。动脉氧分压升高、动脉二氧化碳分压下降。影像学证实术后靶区肺组织有肺膨胀不全、肺不张。结论 BLVR术后肺功能较术前改善。 Objective To explore the treatment effects of one-way valve bronchoscopic lung volume reduction(BLVR) on chronic obstructive pulmonary disease(COPD). Methods Implanted one-way valves in four patients with COPD. Then analysed and compared the data of ventilation, hemodynamics, blood gas before and after implanting. Results After BLVR, forced expiratory volume in one second(FEV1 ) increased by 0.40 liter on average,while forced vital capacity(FVC) increased by 0.31 liter on average. And 6 minutes walk distance increased by 119 meters on average. Partial pressure of oxygen increased and partial pressure of carbon dioxide reduced. Atelectasis was detected in the target area by X-ray examination. Conclusion Pulmonary function improved after BLVR.
