淋巴结外弥漫大B细胞淋巴瘤分子分型的病理学研究

目的探讨结外弥漫大B细胞淋巴瘤(diffuse large B-cell lymphoma,DLBCL)分子分型及免疫组化特点,为临床治疗和预后评估提供帮助。方法采用免疫组化法,标记88例结外原发性弥漫大B细胞淋巴瘤中CD10、bcl-6、MUM1、ki-67的表达并进行分子分型。结果88例结外DLBCL中生发中心B细胞样(GCB)36例,非生发中心B细胞样(non-GCB)52例,GCB组中ki-67的阳性表达率大于75.0%有17例,占47.2%(17/36),non-GCB组中ki-67的阳性表达率大于75.0%有37例,占71.2%(37/52)。结论结外DLBCL以non-GCB多见。

Objective to discuss molecular typing and immunohistochemical properties of extranodal diffuse large B-cell lymphoma （DLBCL） for helping clinical treatment and prognosis evaluation. Methods expressions of CD10,bcl-6, MUM1 and ki-67 in 88 cases of extranodal primary diffuse large B-cell lymphoma were labeled by immunohistochemical method. Results 36 germinal center B-cell like （GCB） cases and 52 nongerminal center B-cell like （non-GBC） cases occurred among 88 cases of extranodal DLBCL. In GBC group, there were 17 cases with positive expression rate of ki-67 over 75.0%, accounting for 47.2% （17/36） ;while in non-GBC group, there were 37 cases with positive expression rate of ki-67 over 75.0% ,accounting for 71.2% （37/52）. Conclusion non-GBC was common in extranodal DLBCL.
