输尿管镜钬激光联合ESWL治疗肾铸形结石

Holmium laser lithotripsy under ureteroscope and extracorporeal shork wave lithortripsy in treatment of kidney staghorn calculi

目的探讨输尿管镜钬激光联合ESWL治疗肾铸形结石的有效性和安全性。方法回顾性分析86例应用输尿管镜钬激光联合ESWL治疗肾铸形结石的病例。结果结石排净率为93．2％。结石排净时间4～38天。术后20例出现尿路感染，2例继发血尿，无输尿管穿孔、粘膜撕脱、输尿管石街形成等并发症。结论采用此联合方法治疗肾铸形结石排净率高、安全有效、损伤小，能有效避免开放手术的痛苦。

Objective To explore the efficacy and safety of holmium laser lithotripsy under ureteroscope combined with extracorporeal shork wave lithortripsy （ESWL） in treatment of kidney staghorn calculi. Methods The clinical data of 86 patients with kidney staghorn calculi treated with above-mentioned treatment were retrospectively analyzed. Results The stone-free rates were 93.2~ and the clearance time to calculi-free was 4-38 days. After operation, Urinary tract infection was found in 20 patients and 2 patients had secondary hematouria. No complications such as ureteral perforation or ureteral mucosa avulsion or formation of steinstrasse were seen after the combined treatment. Conclusion Treatment of kidney staghorn calculi with the combined method has the advantage of high stone-tree rate, safe, effective, less invasire and no suffering of open surgery.
