摘要 目的:探讨64层螺旋CT(容积CT/VCT)对原发性肝癌放疗的指导价值。方法:回顾分析27例经病理证实的原发性肝癌,复查放疗前后肝癌的VCT,并与常规平扫、增强CT进行比较,观察病变检出率、肿瘤靶区体积、放疗前后肝癌VCT变化与放疗剂量的关系。结果:27例中,发现新病灶3例,24例均为单发病灶。增强CT和VCT可以明显提高靶区的准确性。放疗前后肝癌VCT体积变化与放疗剂量存在量-效关系。结论:增强CT及VCT能较好地检出原发性肝癌多发病灶且明显提高靶区勾画的准确性。放疗前后肝癌VCT体积变化有望作为肝癌临床放疗敏感性指标。 Objective:To explore the guidance value of 64 -slice spiral CT (Volume CT/VCT) for primary liver cancer radiotherapy. Methods:Retrospectively analyse 27 cases pathologically confirmed primary liver cancer, review liver cancer VCT before and after radiotherapy, and results of VCT were compared to enhanced CT, scan CT, and observe the lesion detection rate, tumor target volume, relationship between volume changes of primary liver cancer by VCT before and after radiotherapy and radiation dose. Results:In 27 cases, new lesions were found in 3 cases, 24 cases were single lesion. Enhanced CT and VCT can signifieantly improve the accuracy of target volume. There was rela- tionship between the volume changes of primary liver cancer and radiotherapy dose in liver cancer by VCT. Conclusion:Enhanced CT and VCT can detect primary liver cancer and multiple loci effeetivly and significantly improve the accuracy of target outline. Volume changes of primary liver cancer by VCT before and after radiotherapy is expected to be as a sensitive indicator of liver eancer radiotherapy.
