摘要
目的探讨影响颅脑损伤后男性患者性功能的因素。方法对126例颅脑损伤后男性患者的勃起功能进行回顾性调查,采用SPSS统计分析软件对男性功能障碍进行统计学分析。结果本组颅脑损伤男性患者126例,发生勃起功能障碍者18例,发生率为14.3%。颅脑损伤后综合征、GCS、脑损伤部位和应用抗癫痫药物的OR估计值分别为17.020,3.035,11.153和6.141。血清睾酮水平低于正常的8例中,发生勃起功能障碍5例(62.5%);血清睾酮水平正常的118例中,发生勃起功能障碍13例(11.0%),两组相比较,勃起功能障碍发生率差异非常显著(P<0.01)。结论颅脑损伤后综合征、脑损伤的部位、受伤的严重程度、抗癫痫药物的应用和激素水平等因素影响男性颅脑损伤患者的性功能水平,其中以颅脑损伤后综合征的影响最为显著。
Objective To explore the factors influencing the erectile function in the male patients with traumatic brain injury.Methods The inquiry of erectile function was performed by the questionnaires of International Index of Erectile Function-5 (IIEF-5) in 126 male patients after traumatic brain injury, whose clinical data were analyzed retrospectively. The relationship of the erectile dysfunction with some factors including the syndrome of brain injury, serum level of testosterone, injured cerebral regions, GCS and so on was statistically analyzed by SPSS 13.0 software. Results There was erectile dysfunction in 18 of 126 patients after the traumatic brain injury. The incidence of erectile dysfunction was 14.3% in 126 male patients with traumatic brain injury. There were sexual erectile dysfunction in 5 (62.5%) of 8 patients with subnormal serum level of testosterone, and in 13 (11.0%) of 118 patients with normal serum level of testosterone. There was significant difference in the rate of erectile dysfunction between the patients with subnormal serum level of testosterone an ones with normal serum level of testosterone (P〈0.01). The estimated values of OR of syndrome of brain injury, GCS, injured cerebral region and application of antiepileptic drugs were 17.020, 3.035, 11.153 and 6.141 respectively. Conclusions The syndrome of brain injury was the most important factor among the factors influencing the erectile dysfunction including the injury of cerebral limbic system, subnormal serum level of testosterone, severity of cerebral injury and application of antiepileptic drugs etc..
