慢性嗜酸性细胞肺炎的影像学表现(附16例分析)

Imaging diagnosis of chronic eosinophilic peneumonia(with 16 cases reported)

目的:探讨慢性嗜酸细胞性肺炎(CEP)的临床特征及影像学表现,以提高临床诊断水平。方法:回顾分析16例CEP的临床资料、影像学表现。结果:16例CEP胸片显示10例(10/16)X线表现为肺部多发非节段性斑片状阴影,9例(9/16)以中上肺野为主,HRCT显示多发融合性实变10例(10/16),斑片状实变6例(6/16)、磨玻璃影12例(12/16)和条状致密影4例(4/16),11例(11/16)主要分布于中上肺野肺周围部。结论:CEP有较典型的X线与CT表现,结合血液、支气管肺泡灌洗液或组织内嗜酸性粒细胞的增加,可提高临床诊断率。

Objective：To analyze clinical and imaging representation of chronic eosinophilic peneumonia（CEP） retrospectively and to improve clinical diagnosis ability. Methods： This study included 16 cases of CEP, which clinical data were collected and imaging description were analyzed retrospectively. Resnits：In 16 cases of CEP, 10 of them displayed multiple non-segmented patches of pulmonary disease in X-ray plain film, which were mainly located at upper and middle pulmonary field in 9 eases. High-resolution CT（HRCT） showed multiple mixed consolidation in 10 eases（ 10/16）, patching consolidation in 6 cases（6/16）, frosted glass-like shadow in 12 eases（12/16） and strip-like shadow in 4 cases（4/16）. All of these description in HRCT were mainly located at periphery part of upper and middle field in 11 eases（11/16）. Conclusion：Typical X-ray and CT description of CEP, combined with clinical representation, can strengthen clinical diagnosis.
