1483例胸中下段食管癌根治术中游离食管贲门部方法分析

Clinical analysis of 1483 cases of trans-esophagodiaphragmatic ligament dissected esophagocardia to perform middle and lower segment radical esophagectomy

目的探讨胸中下段食管癌根治术经膈食管韧带游离食管贲门部的疗效。方法回顾分析1990年～2008年间1483例中下段食管癌患者经左胸后外侧切口径路行胸部食管癌根治术的相关临床资料。结果1483例中下段食管癌切除率100%,并发症发生率为3.5%(52/1483),无住院期间死亡。结论经左胸食管癌根治术中经膈食管韧带游离食管贲门部方法可使食管下段和胃贲门部位置变浅,视野清晰,操作简便,为胸中下段食管癌根治手术时一种安全有效的手术技巧。

Objective To explore and summarize the effect and experience of via esophagediaphragmatic ligament dissected esephagocardia to carry out middle and lower segment esophageetomy. Methods The clinical data of 1483 patients who trans-left posterior lateral incision thoraeotomy from 1990 to 2008 were retrospectively analyzed. Results The 1483 patients all carried out the procedure. There were no death reported when operating and in the post-operative period. The eomplieational rate was 3.5%. Conclusion When trans-left posterior lateral approach performed radical esophagectomy, the location of lower segment esophagus and gastrocardia will raise, and thus let the operator see it clearer and thus operation make the easier. The dissecting esophagocardia via esophagodiaphragmatie ligament is a safe and effective operation strategy and technique when performing middle and lower segment esophageetomy.
