福氏志贺菌4c型病原学特性及毒力基因检测

摘要 目的:了解上海市闵行区福氏志贺菌4 c型生化特征、对抗生素的耐药性及毒力基因携带情况,为痢疾的防治工作提供依据。方法:对本地区2007年-2008年各肠道门诊监测点腹泻病人中分离到的福氏志贺菌4 c型菌株,用ATB进行生化检测,K-B法进行耐药性检测,PCR扩增进行setlA、set1B、sen、ial和ipaH5种毒力基因检测。结果:36株福氏志贺菌4 c型100%发酵葡萄糖、91.7%发酵甘露醇,27.8%菌株吲哚阳性。对萘啶酸耐药率高达100.0%,利福平、复方新诺明、四环素、氨卞西林的耐药率为97.2%、88.9%、83.3%、83.3%,80.6%菌株耐受一半及以上抗生素。setlA、set1B、sen、ial、ipaH基因携带率均为91.7%、91.7%、100.0%、100.0%、100.0%。结论:福氏4 c志贺菌多重耐药情况较为严重,敏感药物有庆大霉素和第三代头胞类药物。菌株毒力基因携带率高,具有很强的致病性。 Objective:To investigate the biochemical characteristics,antibioticresistance and virulence genes of Shigella flexneri 4c isolates in in Minhang district of Shanghai.Methods:Biochemical identification of the strains was performed using API 32E,serotyping using slide agglutination,antimicrobial susceptibility testing using the Kirby-Bauer method and detection of setlA,setlB,sen,ial,ipaH virulence genes using PCR.Results:The S.flexneri 4c isolates,fermenting glucose and with 91.7% strains fermenting mannitol,were found 100% resistant to nalidixicacid,97.2% to rifampicin,88.9% to SMZ/TM,83.3% to tetracycline,83.3% to ampicillin,80.6% isolates possessed resistance to more than a half of antibiotic species.Presence of setlA,setlB,sen,ial ipaH virulence genes was identified among 91.7%,91.7%,100.0%,100.0% and 100.0% of tested isolates.Conclusion:Strong multidrug resistance of S.flexneri 4c isolates has been found in the present study.Susceptibility to gentamicin and norfloxacin was identified.Presence of virulence genes associated with strong pathogenicity is found in a high proportion of isolates.
