106例骨科医院感染患者围手术期抗菌药物应用分析

Perioperative Use of Antimicrobials in 106 Cases with Nosocomial Infection in Orthopedics Department

目的调查和评价医院骨科I类手术切口医院感染患者围手术期抗菌药物使用情况。方法回顾性调查分析医院骨科2002～2007年I类手术切口发生医院感染的患者围手术期抗菌药物使用情况。结果106例医院感染患者中，85．72％的病例预防用药存在不合理现象；68．87％的病例使用第三代头孢菌素；60．38oA用药剂量过大；54．71％术后预防用药时间〉48h。结论骨科I类手术切口医院感染患者围手术期抗菌药物使用存在用药剂量大、疗程长、首次使用时间欠佳等问题，需进一步规范。

OBJECTIVE To investigate and analyz the perioperative use of antimicrobials in the patients of orthopedics department with nosocomial infection and type I operation. METHODS By taking retrospective study, the patients with nosocomial infection and type I operation from 2002 to 2007 were selected as models to analyz the situation of perioperative use of antimicrobials. RESULTS There was irrational prophylaxis usage in 85.72% cases, such as the usage of third-generation cephalosporins （68. 87%）, overdosage （60. 38%） and long time of prophylaxis usage （54. 71%）. CONCLUSIONS The problems of antimicrohials prophylaxis use during surgical procedures include overdosage, long course and unsuitable time for first time using.
