浆细胞性乳腺炎43例临床分析

Clinical analysis of 43 cases with plasma cell mastitis

目的:探讨浆细胞性乳腺炎的临床诊断、非手术治疗和手术治疗的适应证。方法:回顾性分析43例浆细胞性乳腺炎患者的临床资料。结果:浆细胞性乳腺炎临床表现多样,以乳房肿块多见,辅助检查特征性表现不多。本组13例经非手术治疗而治愈;30例行手术,其中病变乳腺小叶切除术20例,病灶局部楔形切除术4例,乳房单纯切除术2例,脓肿切开引流术3例,慢性窦道及瘘管切除术1例。结论:本病临床表现易与乳腺癌相混淆,外科手术是彻底、有效的治疗方法,应行术中快速冰冻切片及术后石蜡切片病理检查确诊。对于急性期、年龄30岁以下或55岁以上、病程短于3周者,可先行抗炎、局部理疗、乳管冲洗等非手术治疗。

Objective：To investigate the clinical diagnosis and surgical treatment or non-surgical treatment indications for plasma cell mastitis, Methods：A retrospective study was carried out to analyse 43 cases with plasma cell mastitis. Results：The clinical manifestations of plasma cell mastitis varied with no characteristic in auxiliary check, although breast lumps were common. 13 patients in this study were cured by non-surgical treatment, 30 cases received surgical treatment. Among them, 20 cases received breast lobular resection, 4 cases received breast wedge resection, 2 cases received breast resection, 3 cases received abscess incision and 1 case received chronic sinus road and fistula resection. Conclusion： It is easy to confuse plasma cell mastitis＂ with breast cancer. Surgical treatment is a complete and effective treatment, frozen section during surgery and postoperative pathological examination should be performed to confirm diagnosis. For the patients 〈30 years old or 〉55 years old, the course is shorter than 3 weeks who are in acute phase, non-surgical treatment such as anti-inflammatory, local physiotherapy and duct cleaning should be first carried.
