复合性溃疡48例分析

Analysis of 48 cases with complex ulcer

我院90～95年期间经内镜诊断的48例复合性溃疡(CU),占同期1528例消化性溃疡病人的3.14%.其中幽门口变形或狭窄14例(29.1%),其CU的发生与幽门功能异常有关.经活检证实幽门螺杆菌(Hp)感染阳性14例中,11例幽门口圆,开闭好,结合病理检查考虑 Hp感染导致粘膜炎症并形成溃疡.本组内科治疗有效率为89.7%,鉴于Hp感染和溃疡形成有关,并影响溃疡的瘢痕形成,所以根除Hp对CU的治疗及防止复发有重要意义.

Fourty eight cases with complex ulcer(CU) were diagnosed and treated in our hospital during 1990-1995. It accounted for 3.14 per cent of 1528 cases of peptic ulcer confirmed by endoscopy. Of 48 cases 14 (29.1% ) showed pyloristenosis or pyloric deformation, indicating that the complex ulcer was related to pylorus dysfunction. Eleven of 14 cases with Helicobacter pylori(Hp) infection showed that the shape of their pylorus was round and the open-close function was normal. The pathogenesis of the complex ulcer in 11 cases might be related to Hp infection, which then caused mucosa inflammation and produced ulcer.In this group the effective rate of medical therapy for complex ulcer was 89.7% . In view of Hp infection relating to ulcer formation and affecting scar of ulcer, eradication of Hp infection may play a pivotal role in cure of the ulcer and prevention of ulcer recurrence.
