假肥大型肌营养不良心脏功能改变(附406例分析) 被引量:14

摘要 目的:探讨假肥大型肌营养不良(DMD)心肌酶、超声心动图和心电图改变的特点及其临床意义。方法:对406例DMD患者的心肌酶、超声心动图、心电图等检查资料采用临床病例统计学方法进行归纳和分析。结果:所有DMD患者都有不同程度的心肌酶升高,其中肌酸激酶(CK)升高最明显,4~5岁最高,8岁以后随年龄增长而下降;绝大部分患者(383/406例)肌酸激酶同工酶(CK-MB)都升高。行超声心动图检查342例,有异常的患儿89例;行心电图检查355例,有异常的患儿237例,随年龄增长异常率越高。结论:充分认识DMD患者的心脏病变特点,早期诊断、早期预防,对延缓疾病进展和延长患者的寿命有重要意义。 Aim: To explore the changes of blood serum myocardial enzymes, echocardiography, electrocardiograms of the children with Duchenne's muscular dystrophy (DMD). Methods: The data of myocardial enzymes, echocardiogram, electrocardiogram of 406 DMD patients are comprehensively analyzed by using clinical statistical method. Results: The blood serum myocardial enzymes of all patients was higher than normal one' s, especially the creatine kinase(CK). And of all the patients, the level of CK is the highest in these children whose age rang from 4 to 5 years old, and when the children were 8 years old, with the age increasing the level of CK decreased. Myocardial markers such as MB type of crease kinas isozyme(CK-MB) in most patients is higher than normal one' s. 89 of 342 patients had abnormal echocardiography changes, 237 of 355 patients had abnormal electrocardiograms changes, and with the children growth the rate of echocardiography and electrocardiograms abnormalities is increasing. Conclusion: It is very important to full understand the pathological change characteristics of the cardiac in DMD patients and to diagnose and prevent early in retarding the progress of diseases and prolonging the life of DMD patients.
