小梁切开术治疗婴幼儿型青光眼的远期疗效分析

Long-term effect oftrabeculotomy on infantile glaucoma

目的评价小梁切开术治疗婴幼儿型青光眼的远期临床疗效。方法随访19例（31只眼）行小梁切开术治疗的婴幼儿型青光眼患者，观察术后眼压、视力、角膜横径、杯／盘比及手术并发症，回顾性分析手术疗效。结果至末次随访（术后1～80个月），23只眼（74．2％）成功。末次随访时平均眼压较术前降低（t=2．15，P〈0．05）；角膜横径减小（t=2．15，P〈0．05）；平均杯／盘比差异无统计学意义（t=2．0，P〉0．05），但手术成功者的术后杯，盘比较术前减小（t=3．08，P〈0．01）。10例（17只眼）患者配合视力检查，矫正视力在0．02～0．5之间。除20只眼术中前房出血外，所有患者未出现其他并发症。结论小梁切开术治疗婴幼儿型青光眼的手术成功率高，并发症少，应作为婴幼儿型青光眼的首选手术治疗方式之一。

Objective To evaluate the long-term effect of trabeculotomy on infantile glaucoma. Methods Intraocular pressure （lOP）, visual acuity, cornea diameter, cup-disc ratio and postoperative complications of 31 post-operation eyes were following up for 1 to 80 months.Results The success rate of long-term was 74.2%.IOP of post-operation were lower than that of pre-operation （t=2.15, P〈0.05）.Cornea diameter was reduced （t=2.15, P〈0.05 ）.The average C/D ratio difference was not statistically significant （t=2.0, P〉 0.05 ）, but of the postoperative C/D compared preoperative decreased （t=3.08, P 〈0.01 ） in those successful operation cases.Ten patients tmderstood the visual acuity chart, and had correct visual from 0.02 to 0.5.There were no other complications except hyphema during the surgery procecure.Conelusions Trabeculotomy, one of the best choices for the treatment of infantile glaucoma, has a high success rate and fewer complications.
