米索前列醇预防产后出血的临床研究

Clinical Study on Prevention of Postpartum Hemorrhage of Using Misoprostol

目的探讨米索前列醇用于预防产后出血的治疗效果。方法233例妊娠晚期单胎患者分别使用米索前列醇(A组121例,其中阴道分娩69例,剖宫产分娩52例)和催产素(B组112例,阴道分娩63例,剖宫产分娩49例)促进子宫收缩,观察2组患者产时、产后2h及产后24h内平均出血量和用药前后血红蛋白的变化。结果A组产时、产后2h、产后24h累计出血量均显著小于B组(均P<0.05);A组产后24h血红蛋白下降值亦低于B组(P<0.05)。结论米索前列醇促进子宫收缩的作用优于使用催产素,可更有效地减少和预防产后出血。

Objective To study the effect of misoprostol in preventing postpartum hemorrhage.Methods 233 puerperal patients in term pregnancy were selectived and randomized into Group Misoprostol（Group A：69 cases in vaginal delivery and 52 cases in cesarean section）and group oxytocin（Group B：63 cases in vaginal delivery and 49 cases in cesarean section）respectively.The volume of bleeding during the delivery,within 2 hours after delivery and within 24 hours after delivery,the haemoglobin of patients before and after delivery and side effect were observed.Results There was a significant difference between Group A and B in the volume of bleeding during and after delivery（P〈0.05）and the change of the patients＇ haemoglobin before and after delivery（P〈0.05）,both in vaginal delivery and cesarean section.Conclusion Misoprostol is safe and convenient to validly reduce the volume of bleeding during and after delivery and can be used as firstline medicine to prevent postpartum hemorrhage.
