QRS波时限对左主干或三支病变的预测价值

The value of QRS Duration for prediction of the left main or 3-vessal disease in patients with non-ST segment elevation acute myocardiol infarction

摘要 目的分析非ST段抬高型心肌梗死(NSTE-AMI)患者心电图的QRS波时限,通过与冠状动脉造影(CAG)对比,探讨其判断左主干或三支病变的价值。方法测量106例急性非ST段抬高型心肌梗死患者的体表心电图QRS波时限,结合冠状动脉造影结果,分析其诊断左主干或三支病变的敏感性、特异性及相关性。结果QRS波时限与左主干或三支病变具有相关性(OR9.04,p<0.01),QRS波时限≥90ms预测左主干或三支病变的敏感性为88%,特异性为88%。结论QRS波时限是急性非ST段抬高型心肌梗死患者左主干或三支病变非常灵敏的预测因子。 Objective To investigate the effect of QRS duration for Left Main or 3-Vessal Disease (LM/3VD) in patients with Non-ST Segment Elevation Acute myocardiol infarction(NSTE-AMI) by comparing with coronary angiography (CAG). Methods On admission, the clinical factors of 106 patients, who underwent coronary angiography were evaluated. The widest QRS duration were measured on an admission 12-lead electrocardiogram, by which to identify LM/3VD with sensitivity, specificity and dependability. Results QRS duration was associated with LM/3VD, QRS duration≥90ms identified LM/3VD with sensitivity of 88%, specificity of 88%; Conclusion QRS duration was independent predictor of LM/3VD in patients with NSTE-AMI.
